EMBASSY REGISTRATION

Filipinos in Cambodia are encouraged to register with the Philippine Embassy and receive an ID at no cost to the applicant. The contact details given by the applicant will be useful in case the Philippine Embassy issues notices concerning community activity and advisories during emergencies

All information provided to the Embassy will be treated with utmost confidentiality.

The procedure for registering and requesting for an ID is as follows:

FOR ONLINE REGISTRATION

1. Download here and save the editable registration form on your documents/file folder.

2. Accomplish the registration form and save as an Adobe PDF file with your full name as "FILE NAME" (example: Cruz, Maria)

3. Submit accomplished registration form as an attachment to This email address is being protected from spambots. You need JavaScript enabled to view it.  with your full name as "TITLE or SUBJECT OF EMAIL" (example: Cruz, Maria) along with the following scanned documents:

(a) information page of your passport and

(b) 1 x 1 inch recent photo (in .jpg format)

FOR PERSONAL APPLICATION

Duly-accomplished Embassy Registration Form

(a)  information page of your passport and

(b)  2 ID photographs (2.54 cm x 2.54 cm) 

4. Once the application has been successfully submitted via email, the applicant can proceed to the Embassy to request for an ID.  Issuance of the Embassy ID is free of charge.
