(URN - SHIPMENT OF CREMAINS)

 

  • Original Attestation of Death or Death Certificate in Cambodian Language issued by the hospital with English translation and authenticated by the Ministry of Foreign Affairs and International Cooperation;
  • Original Cremation Certificate issued by the funeral parlor;
  • Original Packing and Sealing Certificate issued by the funeral parlor;
  • Original Quarantine Permit for Exportation of Ashes issued by the Ministry of Health - Quarantine Service;
  • Corpse Handover Minute issued by the Ministry of Interior (Department of Technical Science Police - Office of Crime Scene Examination), if available;
  • Copy of the Report of Death issued by the Philippine Embassy, if available;
  • Photocopies of Passport and Visa of the deceased;
  • Letter from the Ministry of Foreign Affairs and International Cooperation to the General Department of Customs requesting permission to repatriate the ashes of a Filipino national; and
  • Fee: USD25.00

(URN - HAND CARRY)

 

  • Original Attestation of Death or Death Certificate in Cambodian Language issued by the hospital with English translation and authenticated by the Ministry of Foreign Affairs and International Cooperation;
  • Original Cremation Certificate issued by the funeral parlor;
  • Original Packing and Sealing Certificate issued by the funeral parlor;

  • Copy of the Report of Death issued by the Philippine Embassy, if available;
