Information on LTO’s Services during the
10th Multi-Agency Service Mission (MASM)
In view of the participation of the Land Transportation Office (LTO) in the upcoming 10th Multi-Agency Service Mission (MASM) on 6-7 April 2019 (Saturday to Sunday), 8:30 am – 5:30 pm, at the Multi-purpose Hall of the Embassy, the Embassy of the Philippines provides the following information related to LTO’s onsite services:
Renewal of PHL Driver’s License (Professional and Non-Professional)
Those with valid licenses or licenses expired for not more than two (2) years may renew during the said dates by presenting the following documents:
- Original driver’s license;
- Duly accomplished application form; and
- Medical certificate (in any format/template) issued by any licensed practicing physician in Cambodia stating that the applicant is “physically and mentally fit to operate a motor vehicle.” If the medical certificate is in Khmer language, an English translation should be included.
Duplicate Renewal of Driver’s License (Professional and Non-Professional)
For lost expired (expired for not more than 2 years) licenses, participants may apply for a duplicate renewal of their license by presenting the following:
- Affidavit of Loss (executed at the Embassy with a fee of USD 25 and will require the submission of a police report, with English translation);
- Duly accomplished application form;
- Original and photocopy of valid passport; and
- Medical certificate issued by any licensed practicing physician stating that the applicant is physically and mentally fit to operate a motor vehicle.
Duplicate Driver’s License (Professional and Non-Professional)
For lost valid licenses, participants may apply for a duplicate license by presenting the following:
- Affidavit of Loss (executed at the Embassy with a fee of USD 25 and will require the submission of a police report, with English translation);
- Duly accomplished application form; and
- Original and photocopy of valid passport.
In keeping with the “one-stop shop” concept of the MASM, the Embassy invited Dr. Charlotte Dizon of Central Hospital to issue the required medical certificates. Her consultation fee is $20. She will be in the Embassy during the following schedules:
- 6 April 2019, Saturday, 8:30 AM - 5:30 PM
- 7 April 2019, Sunday, 8:30 AM - 2:30 PM
(Please note that, as indicated above, LTO will accept any medical certificate from a practicing physician in Cambodia.)
LTO will not be able to renew driver’s licenses that have expired for more than two years. For this case, the driver needs to apply personally in the Philippines to take a written and driving test.
Fees
Applicants are encouraged to pay in Philippine peso. Those unable to pay in Philippine peso are requested to prepare to pay USD 12 to USD 16. The USD rates indicated below are rough estimates, which are subject to change depending on the bank conversion rate on 6 and 7 April 2019:

Other Available Services at MASM
The MASM is a one-stop shop of government frontline services, as follows:
- Overseas Employment Certificate (OEC) processing/renewal;
- OWWA membership;
- NBI clearance issuance/renewal;
- PhilHealth membership;
- PAG-IBIG membership;
- Opening of DBP ATM Savings Account;
- Consultations with the Philippine Statistics Authority (formerly known as NSO) on birth certificates, marriage certificates, and other important civil documents; and
- Embassy consular services: passport renewal, notarization/legalization, civil registry, Embassy ID.
