INFORMATION NOTE

RENEWAL OF NBI CLEARANCE

Based on information obtained from the National Bureau of Investigation, the Embassy informs all Filipinos in Cambodia that holders of an NBI Clearance obtained from 2014 onwards may apply for a renewal WITHOUT accomplishing the NBI Form No. 5 (Fingerprint Card Form), provided that no changes are made on their personal data and upon complying with the following requirements:

• Personal appearance in the Embassy;
• Copy of passport and visa; and
• Fee - USD 25 (for Special Power of Attorney authorizing a representative in the Philippines to file the application with the NBI).

For those with an NBI Clearance issued in 2013 and below, OR those who will apply for an amendment of their personal data, OR new applicants, the requirements are as follows:

• Personal appearance in the Embassy;
• Accomplished NBI Form No. 5 (fingerprint card);
• One (1) colored 2x2 size photo with white plain background;
• Copy of passport and visa; and
• Fee of USD 25 (for certification of accomplished NBI Form No. 5); and
• Fee - USD 25 (for Special Power of Attorney authorizing a representative in the Philippines to file the application)
