The Embassy of the Republic of the Philippines in Phnom Penh will hold Mobile Consular Services in the Provinces of Siem Reap, Banteay Meanchey, and Battambang on 7 – 10 March 2015.
Schedule of Services:
7 March 2015 – 4:00pm – 6:00pm Siem Reap
8 March 2015 – 8:00am – 5:00pm Siem Reap
Eli International School located at #139, Group 11 Krous Village, Svay Dangkum in Siem Reap.
9 March 2015 – 8:00am – 5:00pm Banteay Meanchey
Master’s International School located at Village 3 Preah Punleau Commune, Serey sophon Banteay Meanchey
10 March 2015 – 8:am – 5:00pm Battambang
Every Nation located at Battambang Borey Thmey Mall, Street # 3 Kamakor E Village Sangkat Svay Por, Battambang City.
The Consular Team will provide services such as passport renewal, extensions and notarials (i.e. acknowledgement of employment contract, letter of invitation, etc.)
Overseas voter (OV) registration services for the 2016 national election will also be available.
Registration and issuance of Embassy ID Cards.
Filipinos in these provinces are encouraged to register ONLINE before the scheduled date so that the Consular Team can bring the laminated Embassy IDs.
Please note that the contact details given by the applicant will be useful in case the Philippine Embassy issues notices concerning community activity and advisories during emergencies

The procedure for registering and requesting for an ID online is as follows:
1. Download and save the editable registration form on your documents/file folder

2. Accomplish the registration form and save as an Adobe PDF file with your full name as "FILE NAME" (example: Cruz, Maria)
3. Submit accomplished registration form as an attachment to This email address is being protected from spambots. You need JavaScript enabled to view it. with your full name as "TITLE or SUBJECT OF EMAIL" (example: Cruz, Maria) along with the following scanned documents:
(a) information page of your passport; and
(b) 1 x 1 inch recent photo (in .jpg format)
The Embassy will also bring the passports that are ready to be released. For those who wish to claim their passports, please bring your current passport. Applicants may authorize someone to claim the passport on their behalf. The following are the requirements:
- authorization letter;
- old passport;
- original receipt;
- claim stub; and
- valid ID card of the authorized representative.
