NEW ONLINE PASSPORT APPOINTMENT SYSTEM

FOR PASSPORT TRANSACTIONS

Effective Monday, 17 May 2021, passport transactions (new, renewal and loss passport) will require an online appointment to facilitate passport process and to limit the time of exposure of clients at the consular section.

Booking of appointment must be made three (3) days in advance.  Senior Citizens, Minor children 7 years and below, Pregnant Women, and PWD are not required to schedule an appointment.

HOW TO BOOK AN APPOINTMENT:

• Using a desktop/laptop or mobile phone, go to the DFA Appointment System website: http://www.passport.gov.ph

• Click OK and select “Schedule an Appointment.”

• Read the Reminder and Terms and Conditions before clicking the box and proceeding to select either “Individual” (for one person) or “Group” (for family members) Appointment.

• From the dropdown, select “CAMBODIA” as the Country and “PHNOM PENH” as the Site and click NEXT.

• Select the preferred date and time schedule. Click the reCaptcha box and click NEXT.

• Provide all necessary information required and click NEXT.

• After completing the information, double check the details and click EDIT to amend any incorrect/misspelled information, then click SAVE. Scroll to the bottom, tick “I’m not a robot,” click box after reading the text in red, and click CONFIRM.

• Please check the email provided in the appointment details. Check the Spam folder in case the message cannot be found in the Inbox folder.

• From the email message received, click “CONFIRM YOUR BOOKING.” Check the email again for the Appointment Code and the PDF format of the application form. Read and comply with the instructions provided in the email.

• The appointment code may be used to view the appointment, retrieve application form, re-schedule or cancel an appointment.

IMPORTANT REMINDERS:

• Use ONLY an active Yahoo or Gmail email Make sure that you know your username and password to enable you to retrieve the confirmation of appointment and appointment code emailed by the Passport Appointment System;

• When inputting your telephone or mobile number, do not include the plus (+) sign (ex. 85590746839) to avoid error;

• Input your present address in Cambodia and NOT your address in the Philippines. For Phnom Penh residence, indicate PHNOM PENH in the province;

• Double check all information provided before proceeding to the next step. In case you notice an error after printing your application form, notify the consular officer on your date of appointment regarding the error, for necessary correction;

• To cancel an appointment, you need to re-schedule twice before the CANCEL option will appear in the VIEW DETAILS;

• Print application form on an A4-size paper and bring it and your passport on appointment date; and
